Effective January 1, 2026, AFM Healthcare will implement an Annual Administrative Fee for all non-Medicare patients with commercial insurance. This fee supports the practice by covering non-covered services that are not reimbursed by insurance but are important to maintaining comprehensive, accessible care.


Annual Fee Amount

  • $100 per adult (19–79 years of age) per calendar year
  • $50 per minor (under 19 years of age) per calendar year
  • No fee will be charged for patients aged 80 and older.

Collection Schedule

  • The fee is collected once per calendar year and is due at the first appointment of the year.
  • If your first visit is scheduled mid-year, the fee will be collected at that visit before services are rendered.
  • If you request services (such as prescription refills, medical advice, form completions, or telehealth appointments) before your first visit of the year, the fee will be collected at the time of the request.
  • For new patients establishing care in 2026, the annual fee may be prorated.
  • For patients establishing care in December 2025, the full 2026 annual fee will be collected at registration.

What the Fee Covers (Non-Covered Services)

This annual administrative fee helps offset the cost of non-covered services not typically included under commercial insurance, including:

  • Up to 1 year of maintenance medication refills (when clinically appropriate)
  • Uncomplicated urinary tract infection (UTI) treatment with antibiotics (no visit required unless no improvement)
  • Laboratory review during annual wellness visits
  • Guaranteed same-day telehealth appointments for urgent needs

Is any portion of the administrative fee refundable?

No. Once paid, the fee is non-refundable for the calendar year, even if you transfer care or move.

If I pay my fee in November for my first visit of that year, will I owe it again in February?

Yes. Because February falls in a new calendar year, the annual fee is collected again at the first visit of that year.

Will I still have to pay co-pays, co-insurance, or deductibles?

Yes. Co-pays, co-insurance, and deductibles are separate from the administrative fee and remain the patient’s responsibility.
